Health - Immunizations
Diphtheria
pertussis/Tetanus
(DPT/DT/TD)		 Series of three doses, one of which was recieved age four.
Diphtheria / Tetanus Booster every 10 years 
Measles / Mumps / Rubella One dose after age one, or illness verified by MD.
Measles Second dose before age twelve-may be MMR booster.
Hepatitus B Those born between 9/2/88 & 9/1/92 must recieve 3 doses within 30 days after 12 th  birthday.
Varicella
(chickenpox)		 If born between 9/2/88  & 9/1/92 , must have by age 12 or proof of illness.
If you are tranferring into our district from an area where tuberculosis is present, you will be interviewed by the school nurse and may require a skin test.

Immunization records must include type, date administered, and be validated by signature of health professional or health department. If you have any questions, please contact your school nurse.
